- If you are an HFA affiliated publisher and your tax information has changed, please use this form.

Mechanical License Request Form

- If you are a licensee making less than 2,500 copies, we recommend you complete your licensing transaction through our online service Songfile. If you are making more than 10,000 copies, or are an affiliated publisher that wishes to notify us of an unlicensed recording, please use this form.
