If you are a publisher, complete and sign a Change of Address Notification Form (below), AND a W-9 Tax Form for U.S. companies OR a W-8 Tax Form for non-U.S. companies, so we can update your address accordingly. If you are a licensee, just complete and return the Change of Address Notification Form.
